Scan it with the direct cut feature on a CM/DX/SDX ScanNCut machine
Add your own cut outline or a shape from you're scanncut machine and cut it out.
App size is 3.75 x 4.75 but can be resized.
 
Use this design for sublimation -
Print the design on sublimation paper with a sublimation printer & Ink
Apply to a sublimation blank and follow the heat setting instructions specified with your sublimation paper.
